Radical Nephrectomt and Simultaneous Cryoablation for Bilateral Multiple Renal Cell Carcinoma in Von Hippel-Lindau Disease

Abstract
Multicentric and bilateral renal cell carcinoma (RCC) develops in approximately 50% of patients with von Hippel-Lindau (VHL) disease, and its treatment still remains a challenge for surgeons. Current surgical options include bilateral nephrectomy with renal replacement therapy or partial nephrectomy. But in this situation, cryoablation can be a novel nephron-sparing option. Here we present a case of bilateral RCC in VHL disease managed by right radical nephrectomy due to large size and adhesion to adjacent organ, and simultaneous cryoablation for left kidney. With both modalities, we had the advantage of easy identification and approach for tumor, in addition to the benefits of cryoablation including minimal invasiveness, organ preservation, performance ease, and avoiding renal ischemia. During the eighteen months of follow up, there was no evidence of tumor recurrence.
